沒有登不上的高山:在老撾消除小兒麻痺症

Once finished at the vaccination site, Daeng Xayaseng, Maternal and Child Health Manager, and her team go door to door in the village to ensure every child has received a polio vaccination in Nampoung village, Vientiane Province, Lao PDR.  .  A 10-day polio vaccination campaign in 13 provinces in Lao PDR was undertaken in March 2018 to vaccinate about 460,000 at-risk children aged 0 months to 5 years of age. This is in response to a polio outbreak in 2015 and 2016. 

More than 7,200 volunteers and 1,400 health workers ensured that all children within the 0 to 5 age group in the selected provinces and districts receive the two drops of oral polio vaccine. Children also received routine immunization vaccines. 

Throughout the campaign period, vaccines will be provided at health centres, as well as district and provincial hospitals, kindergartens, markets and bus stations. Furthermore, there will be outreach sessions, including house-to-house visits to identify and vaccinate children in high-risk areas, and special sessions in rice fields and other plantation areas. 

As of May 2017, Lao PDR is officially no longer infected with circulating-vaccine derived polio virus type 1 (cVPDV1). After the last outbreak in 2015 and 2016, the country has now been without cases for over 24 months, with the last case reported in January 2016.

在老撾中部一個偏遠的村莊,達英‧沙雅森與衞生外展團隊長途跋涉,尋找錯過接種小兒麻痺症疫苗的兒童。

達英‧沙雅森靠電單車和雙腳在崎嶇不平的鄉間穿梭,為在偏遠村莊居住的兒童接種疫苗已有15年。

工作雖然辛苦,但她保護兒童的決心堅定不移。她說:「只要兒童有需要,無論要用多長時間,我們都會為他們接種疫苗。我們會堅持工作,直至每名兒童都成功接種疫苗。」

今日,她與外展團隊來到距離老撾首都以北4小時路程的南邦村,為當地兒童接種小兒麻痺症疫苗。

偏遠和貧窮的少數民族社區與赫蒙族村一樣,是最容易傳播疾病和病毒的地區。當爆發危險病毒時,衞生團隊及時為最需要的人提供支援對控制疫情至關重要。

沙雅森說:「提供衞生服務的時間取決於季節,現在是稻米收割季節。村民都是農民,所有人會去離家很遠的田地工作,所以服務時間要根據社區的要求作出調整。 如果他們說下午3時,我們就會在那個時間來。如果他們說晚上有空,我們也會按時出現。」

兩名男童在家門外接種疫苗。他們並非主動前來接種疫苗,而是衞生外展團隊主動在南邦村尋找他們。

外展團隊首先在村的中心設立疫苗接種點。村長透過響亮的喇叭用赫蒙語告知父母帶孩子來接種疫苗。幾分鐘後,有父母背着年紀較小的孩子及拖着年紀較大的孩子紛紛趕來。

沙雅森說:「我一直做這份工作,持續了15年。今日,我們的外展團隊來到這裏為兒童接種小兒麻痺症疫苗。我們會逐家逐戶接種,確保不會遺漏任何一名兒童。」

當接種點完成疫苗接種後,外展團隊就會出發逐家逐戶尋找尚未接種的兒童。如果某個社區接種疫苗的人數過少,疾病和疫情爆發的風險就會增加。兩年前,附近村莊就曾發生這種情況,最終導致悲劇的發生。

完成一天的工作,達英‧沙雅森與外展團隊步行穿過村莊踏上返程,他們已為幾百名兒童接種疫苗,包括小兒麻痺症疫苗和可預防其他疾病的疫苗。

沙雅森與外展團隊下定決心要確保不再發生此類疫情。

沙雅森說:「我們不希望這裏再次爆發小兒麻痺症,因此我們必須為所有人接種疫苗。在偏遠社區,當務之急就是為每名兒童接種疫苗,確保他們都得到保護。」

聯合國兒童基金會(UNICEF)與世界衞生組織合作為老撾政府提供支援,為約50萬名5歲以下兒童接種挽救生命的疫苗。目前已有超過7,200名義工和1,400名醫護人員展開行動,他們像沙雅森及外展團隊一樣,從事接種小兒麻痺症口服疫苗及麻疹、德國麻疹疫苗等工作。

達英‧沙雅森說:「我為從事這份工作感到自豪。」

幾個小時後,工作完成了。

外展團隊整理收拾後,沙雅森說:「我對工作感到非常滿意,並為從事這份工作感到自豪。它讓我為社區服務,並盡我所能為大家提供支援。」
